Accessing your Checklist forms

The following forms are accessible by logging into your . You will now use your ¿­ÐýÃŹÙÍø email to log in. If you have any questions, please contact admissions@bennington.edu.
 

If you don't have access to your ¿­ÐýÃŹÙÍø email, please contact the Help Desk at helpdesk@bennington.edu.

Form Deadlines

May 25
  • Round I Course Registration Form DUE (required). This form will be available May 8. 
June 1
  • Register for your ¿­ÐýÃŹÙÍø email account (required)
  • Contact and other information due (required)
June 15
  • Roommate questionnaire due (required)

  • Course registration due (required). This form will be available in the first week of June.
     
  • Accommodation request due (optional) - Please work with a medical provider to fill out this request form if you plan to request specific academic or housing accommodations in college. Learn more about Accommodations and Support at ¿­ÐýÃŹÙÍø.
    • Enrolled students and parents/guardians are invited to contact us at accommodations@bennington.edu if they have questions about academic accommodations and support or would like to discuss their (or their student's) particular learning needs.

  • Health information due (required)
    Early this summer you will be emailed a notification from Practice Fusion, our Health Center's software. The email will have a link that will allow you to create a patient portal. Once in the portal, you can fill out all required health and insurance information. The required information includes:
    • Health history
    • Immunizations record
    • Documentation of varicella (chickenpox)
    • Tuberculosis (TB) screening document 
      If any questions on the TB screening are answered Yes, an additional tuberculosis (TB) risk assessment form is required
    • Consent for medical treatment
    • Release of records
  • Proof of health insurance due (required)
    All ¿­ÐýÃŹÙÍø College students are required to have health insurance and to provide policy information to the College. This will be done via Practice Fusion, our Health Center's software. International students will receive separate information about a required health insurance plan.
  • Transportation options form due (optional) 
  • First-year credit transfer form due (optional)
    Please fill out this form if you have earned credits that you would like to have reviewed and transferred to ¿­ÐýÃŹÙÍø. Read more about ¿­ÐýÃŹÙÍø’s undergraduate transfer credit policy.

September 1
  • As part of ¿­ÐýÃŹÙÍø's comprehensive wellness program, we will require incoming students to complete some online courses.  We have partnered with 3rd Millennium Classrooms to offer online prevention and intervention courses.
